Public Notice of Health Care Facility Closure

As of June 1st, 2026 (the "Closure Date"), Dry Creek Surgery Center (the "Company") ceased operations at 125 Inverness Dr East, Ste 150, Englewood, CO 80112.

As of the Closure Date, the Company no longer participates in the Medicare Program (Title XVIII of the Social Security Act), and the related agreements with the Secretary of the United States Department of Health and Human Services will be terminated in accordance with the provisions of the Social Security Act.

The Medicare Program will not make payments for services furnished to patients on or after June 1, 2026.
